About the Job

Cosan Group is seeking a Care Coordinator to support a patient panel completely from home. The role focuses on outreach, health coaching, and collaboration with clinical teams, with a strict HIPAA-compliant home office and a 100% WFH schedule (Mon-Fri, no weekends).

You’ll manage 400–450 patients, perform 50–60 daily outbound calls, and document interactions in the care coordination platform while ensuring privacy and accurate reporting.
